• Clinical Case Study for Assessment Task 2Clinical Case StudyMrs. Helena Jones is a 48 year old lady with a 13 year history of multiple sclerosis. Helena manages her chronic conditions at home, her General Practitioner (GP) has contacted the community nurse for a review of her chronic conditions under the Team Care Arrangements.Past medical historyHelena has a 13 year history of Multiple Sclerosis. Since then she has developed severe weakness in her left leg and foot, requiring a walking stick to mobilise due to difficulty in walking, she has extreme numbness in her right hip, difficulties in holding objects in both hands and reports sensations of “spinning and dizziness”. Since 2 years ago she has progressively declined with numbness and weakness in both legs and hands, is incontinent of urine and is losing sight in her right eye. She has complete loss of balance and can no longer walk or stand and is in a wheelchair for most of the day, she has severe vertigo and muscle weakness. She can stand with assistance to get into the shower in the morning. She has difficulty swallowing and has had a Percutaneous Endoscopic Gastrostomy (PEG) Feeding tube inserted 1 year ago and receives feeds of ‘Jevity with Fibre’ 60 mL/hour via a Kangaroo pump from 8am to 8pm, no feeds overnight. She uses incontinence pads. Six months ago she had increasing thirst, hunger and was urinating more than normal. She was ordered an oral glucose tolerance test with a result of 6.7% (HbA1c) and her GP diagnosed her with type 2 Diabetes Mellitus.
