I need help in Access database 2010

Can any one help me do these two cases by 10 pm tomarrow night(11/20/2012). Please do work  in MicroSOft Access 2010..(Make sure no 2007, ONLY 2010).…  I have attached the files, please look at the img-0582 and img-0583 is one case 5,  and IMG-0584 is seperate case…

So you case 5 you need the img-0583 and 0584

for case 6 you need the IMG-0584
